List forms

GET/api/v1/workspaces/{slug}/certificates/{id}/forms

List a certificate's request, unlock, and correction forms

Returns all three form kinds for a certificate. A kind the certificate has never configured comes back with an empty field list and enabled: false, so clients never have to branch on absence.
